Registering With The Practice

Contact the surgery and collect a registration form. When you have completed all of the details please bring it back to the surgery. When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

You will be asked for proof of address and photo ID. The proof of address should be valid within the last 3 months. 

We cannot register a child without a registering adult (parent / main legal carer)

if you wish to register  an elderly relative that you care for or any one you are a carer for that lives with you ,We encourage, where possible, that Elderly / dependent relatives living with the main carer should be registered where the main carer is registered. From our experience this is more conducive to better patient care.  if this is not possible then please discuss with a member of our team.

You may print off a registration form and heath questionnaire, fill them out and drop them in to  the practice.
