Register with the surgery
We use an online service called Register with a GP surgery that makes it easy to register with this us.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

Dear Patient,
we want to make our communications clearer and simpler – so you and anyone supporting you can access and understand the information we send you.
Please let us know:
if you find it hard to read our letters
if you need someone to support you at appointments
if you need information in alternative formats (such as Braille, large print or easy read)
if you need a British Sign Language interpreter or advocate
if you lipread, or use a hearing aid or other communication tool
If you need a language Interpreter for your appointment, then please let a member of staff know before the appointment .A longer appointment will need to be booked. The Interpreter will then be contacted at the start of the appointment.
if you need support to attend appointments. This could include giving you longer appointments and appointments at times when an advocate or carer can attend with you

We want to make sure you get the most out of our services. That means we need to know which ways of communicating work best for you and, if applicable, for your family or carers.
Please let the receptionist know what we can do to meet your needs when you arrive for your next appointment or contact us by telephone 0113 2535134 or visiting our website, the address is gildersomehealthcentre.nhs.uk and fill in a contact form.
We will then add your communication need to your medical record and review each time you attend. Please let a member of staff know of any changes.
Practice Boundary
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 8am to 6:30pm.
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
